At Allure Wellness Center, we are committed to protecting your health information while providing personalized, patient-centered care.

Your Rights

You have the right to:

  • Get a copy of your paper or electronic medical record

  • Request a correction to your medical record

  • Request confidential communications (e.g., preferred phone or mailing address)

  • Ask us to limit the information we use or share

  • Get a list of those with whom we’ve shared your information

  • Get a copy of this privacy notice

  • Choose someone to act for you (e.g., medical power of attorney)

  • File a complaint if you believe your rights have been violated
     

Access Your Medical Records

You may request a copy of your medical record at any time. We will provide it within 30 days and may charge a reasonable, cost-based fee.


Request Corrections

If you believe your information is incorrect or incomplete, you may request a correction. We may deny the request but will provide a written explanation within 60 days.
 

Confidential Communication

You may request that we contact you in a specific way (e.g., phone, email, or alternative address). We will accommodate all reasonable requests.

Your Choices

You have control over how we share certain types of information.
 

You can tell us your preferences for:

  • Sharing information with family, friends, or caregivers

  • Disaster relief situations

  • Inclusion in a facility directory (if applicable)
     

If you are unable to communicate your preferences, we may share information if it is in your best interest or necessary to prevent harm.


We will NEVER share your information without written permission for:

  • Marketing purposes

  • Sale of your personal health information

  • Most psychotherapy notes


Fundraising Communications

We may contact you about wellness programs or clinic updates, but you can opt out at any time.

We typically use or share your information to:
 

Treat You

We share your information with healthcare professionals involved in your care.
Example: Coordinating care between providers for hormone therapy or chronic condition management.

 

Run Our Practice

We use your information to improve services, manage operations, and communicate with you.
 

Bill for Services

We share necessary information with insurance providers or billing entities.

Other Ways We May Share Information

We may share your information when required or permitted by law, including:

  • Public health and safety reporting

  • Preventing disease or adverse medication reactions

  • Reporting suspected abuse or neglect

  • Health research (when permitted)

  • Compliance with federal and state laws

  • Organ and tissue donation requests

  • Working with medical examiners or funeral directors

  • Workers’ compensation claims

  • Law enforcement or government requests

  • Legal proceedings (court orders or subpoenas)
     

Substance Use Disorder Records (42 CFR Part 2)

If applicable, substance use disorder records are given extra protection. We will not share this information for legal proceedings without:

  1. Your written consent, or

  2. A valid court order and subpoena

Your Right to Limit Sharing

  • You may request limits on how we use or disclose your information

  • We are not always required to agree, but we will consider your request carefully

  • If you pay out-of-pocket in full, you may request that we do not share that information with your insurer

Accounting of Disclosures

You may request a list of disclosures made over the past six years (excluding routine care, payment, and operations).

  • One report per year is free

  • Additional requests may incur a fee

Our Responsibilities

At Allure Wellness Center, we are required to:
 

  • Maintain the privacy and security of your protected health information (PHI)

  • Notify you promptly in case of a data breach

  • Follow the practices described in this notice

  • Not use or share your information beyond what is described without your written permission
     

You may revoke permission at any time in writing.

If you access your health information through our secure online portal available via myallurewellness.com, you can:

  • View records

  • Request appointments

  • Communicate securely with our team
