Federal Review by the Office for Civil Rights

The U.S. Department of Health and Human Services Office for Civil Rights (OCR) reviewed the reported incident after receiving the breach notification submitted by our organization.

After examining the matter, OCR issued an official letter confirming that the case has been closed without further action. 

This means the review has concluded and no enforcement measures were taken against our clinic.

Review by the Indiana Attorney General’s Office

The Indiana Attorney General’s Office also reviewed the reported matter as part of its oversight responsibilities regarding data privacy.

After evaluating the situation and the information provided, the Attorney General’s Office also closed its review of the matter.
